New Consultative Service Request Form

We are pleased to announce the Consultative Service Request Form as the newest addition to eService. As of November 29, 2012, all formal consultative service requests, including key systems phone conferences, must be submitted by logging onto Online Order Entry and completing the new Consultative Service Request Form.  If the situation calls for onsite consultation, the same eService form will need to be completed.  Once submitted, the form will automatically route to the Consultative Requests Queue within DFIS for it to be reviewed, approved, and assigned to the appropriate team members to coordinate the meeting.

By launching this new eService tool, we will continue to achieve standardizing our internal processes, increase the visibility and tracking ability of consultative services, and increase Online Order Entry usage. Attached you will find screen shots of the new feature.
